If you have already scheduled your appointment, print out the forms from our Patient Forms Page, fill them out, and bring them with you to your appointment. Dr. Dan Thieme and his staff are committed to providing you with the very best care for your eyes. We take the time to do it right, and give you all the options available. For general eye exams, eyeglasses, contact lenses, comprehensive medical eye examinations, surgical consultations, minor eye emergencies (infections, objects in eye, etc.) call us anytime at 888-2200, or in Payette/Ontario 642-4434.
